it was the longest 2 minutes of my life, waiting for them to announce the results of the students whom have passed. we were all gathered at around 12.45pm, thursday. my name was among the last to be called.i was holding my breath and sucking in my stomach. it was such a relief when my name was finally mentioned. ALHAMDULILLAH. our exams was not without casualties. 2 of the brothers have to re-sit for the exams in 6 months time. they're good students. maybe Allah wants to give them this challenge to make them stronger and more strong-willed to face life's challenges ahead.
lepas tu i happily gave out my wedding cards to my friends. nanti i'll scan the card and post it here. syahadah and sibah were busy giving out their cards too. orang lain punye tak siap lagi.
petang tu there was a program for the graduating students (my batch and a few sciences students). terharunya dr melor went to greta lengths to organise the program so that we'd remain in the tarbiyah system. i cant really imagine myself outside of it. i'd be lost. there was amaanats form ustz fadzil konsis, dr latif and dr munawwar hatta. best sangat. it put things back in persepctive.
then when the program ended, our juniors in the usrah system came and performed nasheed for us, prepared a slideshow presentation of the activities done together and gave us all personalized gifts. i menangis2 terharu. huhu. asyek terharu je sejak akhir2 nih.
i look forward to the days ahead.

finally final
the final, ultimate most dreaded day has come and will be over in a few hours.
my exams started at 3pm. and ended at 5pm.
long case i got a medical case. the patient, a 60 year old gentleman had DM, HPT, IHD post CABG and asthma. and it all started with a history of urinary tract stones and acute urinary retention. nganga jap td nak cari chief complaint. mule2 i thot it was surgical until i was halfway thru the history. my examiners were dato'sapari (medical- he was the chief examiner since it's his field), external examiner paeds, prof kyaw (surgery) and dr murad (ong). i present je pape. i dont think i impressed anyone. but i think i was passable because i could answer simple questions. prof kyaw asked me about the stones. dr murad couldnt asked me anything since the patient's reproductive tract was external.
then for the short cases, mr jun asked me to examine an indirect inguinal hernia, the ong external examiner asked me to examine an ovarian mass while the paeds ext examiner asked me to confirm whether the little girl had heart failure or not. baik giler derang.
best thing is, no one has to be re-examined on thursday. alhamdulillah. coolness.
we're one step away.

post exams syndrome
i've just finished my surgical theory exams.
i tackled the MCQ'S like i always do. as much as i possibly can. setakat ni my "strategy" has worked for me.
OSCE keluar trochar for spc, proctoscope for haemorrhoid banding (i got this one wrong), IVU showing bilateral hydonephrosis and hydroureter, ERCP with dilated CBD and stones.
i nganga jap tengok trochar tu. never seen it before. and then looked at mr jun for help. he didnt give me the answer. i guessed it was for SPC and he said "ye la tu". yay, baru boleh jawab the questions hehe. thanx mr jun.
PMP case 1 MVA with splenic injury and shock, case 2 prostete carcinoma. asyek keluar je prostate ca tapi i asyek tak dapat jawab management dia. hopefully when i see a real case nanti, i'll be competent to manage the patient well.
k, pray that i can do my long case and short case well this wednesday. jzkk.
(can't wait to go see my sisters)

i was listening to these words while i was revising yesterday and thought it was nice to digest and share with others. banyak kali je dengar. yesterday, i made the effort to type it out.
“Every vessel, every container, will only pour out what is contained in it, that is very obvious.

2 of my friends are pregnant.
my best friends are no longer single and available. yay! except roomie that is.

ortho revision
today i had to present a case for the ortho revision class with mr hafiz. one brother one sister had to present. a friend of mine who got ditinction was supposed to present. logically, that's just because they would be the person who remembers the most about orthopaedics and it would be more beneficial for everyone.
but my fren had her exams on wednesday afternoon and was afraid that she couldnt clerk a patient in time. since my clinical exams finished on tuesday, before my exams, she asked if i could replace her (i was suggested by 2 of my verrrrrrry good and thoughtful best friends. hmm). being the kind person that i am (hehe), i susah nak say no but i thot i'd try to find someone else to replace me.
rupe2nye, harapan tinggal harapan. no one wanted to take up the job or even help me do it. i tak ingat hape pun pasal ortho except that its about bones and accicents and a few other headings je.
pergilah clerk sorang2 pada pagi rabu semalam and mintak belas ihsan mane2 4th year yg ada di wad. and mengadu pada pak din and musab. they asked me to do a diabetic foot case sebab konfem kuar pro. so pergilah mencari patient pempuan di was 2A. i clerked this one indian lady whose foot was quite badly looking. her daughter was very cooperative. the patient herself macam layan separuh hati. it was oredi lunchtime by then and i had to attend asma's bachellorette party at ECM hehe. asma' belanja at johnny's before she becomes a mrs.
then petang nak sambung examine patient with faizah, but the patient buat2 tidur so we frantically looked for another case at wad laki2 plak.
alhamdulillah there was this 61 year old malay pakcik who was very cooperative. so we could clerk and examine him fully setakat mane yg ingat.
yesterday night, my frens tolong me perpare the case. saaayang faizah and sibah and aida :)
today punyelah berdebar sebab kene present in front of the whole batch. besenya in a small group je. alhamdulillah mr hafiz was nice and gave enlightening commentaries and tips for pro. it wasn't that bad. hopefully everyone got something out of it.

my exams.
yay i dah habes my paeds clinical exams yesterday.
i got long case bronchopneumonia (ddx tb) becoz of fever and chronic cough for 1month with dr fadzil.
short case steroid resistant nephrotic syndrome with cushingoid features with dr aye aye (question: look at this boy and examine him accordingly). kene examine steroid toxicity. good thing dah buat ngan dr zabidi hari tuh. he had moon facies and bilateral leg oedema. yg lain sume tade.
OSCE kuar gambar conjunctival haemorrhage, perianal desquamation, rashes on the lower limb extensors, rashes on the back and barium meal of duodenal atresia. sume org ada macam2 jawapan. 2 minits nak pikir. kring loceng dah bunyi. hentam sajelah whatever pun. haha.
PMP kuar diabetic ketoacidosis and hydronephrosis with UTI secondaru to genitourinary tract anomalies.
i had a rather bad episode of psychosomatic symptoms this time around. now dah abes the triger, boleh lah relax kejap for 2 months (yeke?). i think worries of PRO lagi teruk kot.

im clinical exams
after the exams today i felt numb. agaknye sebab anxious sangat before that.
it all ended at around 11.30am.
for my long case, i got a left deep vein thrombosis (dvt) of a 52 year old malay gentleman with underlying diabetes melitus and hypertension. his only complaint was of a painless and progressive swelling of the left calf. alhamdulillah there were no symptoms of pulmonary embolism. i've read about DVT but not actually seen any case throughout my 7-week posting. setakat baca dalam buku and pernah jumpe case dulu2. so i tried to ask for risk factors of DVT such as prolong immobilization, trauma etc but he didnt really have any risk factors. he claimed that his diabetis and hypertension was well-controlled.
waktu tengah clerk tu ada kak farah (houseman) reviewing the patient in the opposite bed so i asked her about this patient. at least i tak tengok patient's case notes which were on the table. so she told me a bit about the case.
then i had to present to Dr Hilmi who is quite nice and not scary. i said i'd give thrombolytic therapy to the patient to lyse the clot and dr was hmmming and said "really?". i degil gak nak bagi thrombolytic and anticoagulant. sebenanye tak ingat sangat management dia. tak pe i'll look it up after this.
then i had to wait for a while to be taken for my short case. maka datanglah kak nik (the same lecturer i got mase 3rd year. dulu i got a respi case and managed to diagnose it as bronchiectasis tapi presentation terabur giler).
this time she asked me to examine the abdominal system of a young girl. she was jaundiced and pale. i couldn't detect the hepatosplenomegaly. but there was ascites. i knew that all those signs were consistent with hemolytic anaemia. at that time my mind just went blank. i couldnt remember all the causes yang banyak itu including thalassaemia or even SLE. terus dr bebel2 satu wad dengar. she asked me basic questions such as examintaion techniques etc. sedih2.
i dunno how much she gave me but the total marks(SC=LC) accumulated to a pass alhamdulillah.
i will so not sia2 kan my next posting.
yay to delivering babies and meeting expectant mommies! and also the notorious anti-islamist!
